Read any GA publication in the UK and airspace infringements feature regularly.

Industry wide initiatives include the Airspace & Safety Initiative (ASI) which has recently launched an interactive guide to ATS outside Controlled Airspace. This can be viewed at www.airspacesafety.com. (The ASI is a joint initiative between the CAA, NATS, AOA, GA and MoD).

Pilots, of any ability, should STOP and THINK about safety, including the potential risk of airspace infringements, when planning a flight and while flying.
